Genealogical Records are birth, death and marriage vital records that occurred:

  • (Birth) more than 80 years ago
  • (Death) more than 40 years ago
  • (Marriage) more than 50 years ago

This type of record is normally needed for family research, but if the person or persons that are on the record are still living, you will need to apply with the Non-Genealogical NJ Vitals Application.

Ordering a Genealogical Record

The request cannot be processed as a genealogical request if the person/person's named on the record is still living or the event was more recent than previous to the what is listed above.

The Rahway Office of Vital Statistics holds records dating back to the early 1800's. You may apply in person or by mail.

The following must be sent with your application:

  • All required copies of identification for proof of identitydownload
  • Copies of documents proving your relationship to the person/person's named on the record

Your application will be returned if you do not send in all required documentation 

Apply In Person:


The applicant will receive the certified record at the time of application

You will need to provide:

  • A valid photo ID (not expired)
  • The NJ Vitals Application completed with all information listed above
  • Check or Money order made out to The City of Rahway. ($10.00 per certified copy)


These are Original Certified Copies with the raised seal


All checks are to be made out to:

Apply By Mail:

You will need to mail in:

  •  A photo copy of a valid photo ID (not expired)
  • The competed NJ Vitals Application with all the required information listed above
  • Check or Money order made out to The City of Rahway. ($10.00 per certified copy)
  • The address that you would like us to mail the Certificate back to

These are Original Certified Copies with the raised seal

We will then mail the Certificate to you via USPS. We will issue the Certificate the same day we receive the application, required documentation and fee. If you would like the Certificate overnight; you must include an overnight envelope with your application, required documentation and fee.


Please do not send in original ID's or documents. 
Only photo copies are required.
